If you are placing your first order with BDS:

Tests can also be directly ordered from this webpage or through the contact methods mentioned above.

Test Order Form Instructions for Current or Established Customers:

1. Customer ID:

   . Input your Customer ID which can be found on your latest invoice or statement.

   . By doing this, there's no need to input your name, agency, address, or phone number.

   . If your Customer ID isn't on hand, simply leave that field empty. However, you'll need to fill in your personal and agency details.

2. **Email Address:**Provide your email so BDS can send any future updates or important information.

3. Selecting Tests:

   From the provided dropdown boxes, select the test(s) you wish to order.

4. Number of Diskettes:

   Indicate how many diskettes you'd like to order.

5. Test Quantity per Diskette:

   Specify if you'd like 25 or 50 tests on each diskette.

6. Diskette Version:

   Choose between MS-DOS or Windows diskettes. If you're uncertain, please call BDS at 602-234-3506 for guidance.

7. Submit Your Order:

   Once all fields are filled, click the "Submit" button to finalize your order.
