How To Order Tests for Established Accounts

If you already have a BDS account and a designated account ID number:

For New BDS Customers:

If you are placing your first order with BDS:

  • We highly recommend visiting ourNew Client Orders page to review the "Instructions for New Customers" prior to making your order.

Tests can also be directly ordered from this webpage or through the contact methods mentioned above.

Test Order Form Instructions for Current or Established Customers:

  1. Customer ID:
    • Input your Customer ID which can be found on your latest invoice or statement.
    • By doing this, there's no need to input your name, agency, address, or phone number.
    • If your Customer ID isn't on hand, simply leave that field empty. However, you'll need to fill in your personal and agency details.
  2. Email Address:Provide your email so BDS can send any future updates or important information.

  3. Selecting Tests:

    From the provided dropdown boxes, select the test(s) you wish to order.

  4. Number of Diskettes:

    Indicate how many diskettes you'd like to order.

  5. Test Quantity per Diskette:

    Specify if you'd like 25 or 50 tests on each diskette.

  6. Diskette Version:

    Choose between MS-DOS or Windows diskettes. If you're uncertain, please call BDS at 602-234-3506 for guidance.

  7. Submit Your Order:

    Once all fields are filled, click the "Submit" button to finalize your order.
